Learning Collaborative for Affordable Senior Housing and Services

The Learning Collaborative is a national initiative to bring teams of housing providers and service providers together to develop and test strategies to coordinate and deliver health and supportive services to residents in affordable senior housing properties. These strategies will support older adults in their ability to safely age in place in their homes and communities. SET has partnered with the Milwaukee County Department on Aging to develop a strategy that will improve the quality of life for senior residents in Milwaukee county public housing by augmenting the Lapham Park Venture.

Lapham Park Venture

SET is one of four founding partners and is the lead agency in providing onsite case management and coordination at the Lapham Park public housing apartment building. The Lapham Park Venture is a public and nonprofit partnership whose mission is to create a supportive continuing care community for low-income older adults to comfortably age in place.

Goals:
  1.  to promote quality of life, health, and housing stability among Lapham Park  residents in an aesthetically pleasing environment
  2.  to nurture a sense of community, pride and ownership, empowerment, and self-advocacy
  3.  to maintain a collaborative partnership to foster a sense of program unity and cohesiveness in the service team
  4.  to create and replicate a model service program for the City of Milwaukee and other housing communities

Milwaukee Succeeds

Milwaukee Succeeds is a collaborative effort created to ensure educational success for all Milwaukee children. SET is a Network Member Organization in this long-term partnership and is dedicated to bringing about change to Milwaukee schools. SET Representatives sit on two of the four goals, and participate in the Operations Team and Network Leaders Committee.

Early Childhood Readiness Team:

SET is the co-leader committed to ensuring all children are ready to enter school. We are part of a Parent Education and Support Network. The team goals include…

  • Improve vaccination rates for children aged 19-35 months in Milwaukee in an effort to reduce the incidence of vaccine-preventable illnesses.
  • Ensure investment in early care and education that meet high quality standards and are available to all children birth to age 5
  • Align child-serving systems in Milwaukee to empower and engage parents in their child’s healthy development so children are ready to succeed in school and life
Student, Family and Community Support Team:

As a member of this team, SET recognizes that there are economic difficulties facing families. This collaborative provides support to families, children and young people to ensure they are healthy, and supported socially and emotionally, so they can contribute responsibly to the success of the Milwaukee community.
