The Housing Case Management Program provides stability and a higher quality of life to adults living in public housing in Milwaukee. SET’s nurse case managers and social work case managers focus on assisting seniors and residents with disabilities so that they can be self sufficient.
The Housing Case Management Program :
Advocates for various services and resources on behalf of residents.
Identifies residents at risk due to substance abuse and/or mental illness.
Helps senior residents in public housing remain independent.
Assists residents with obtaining government aid.
Creates and implements support groups for residents.
These services make a difference in the every day struggles residents face in public housing. For example, approximately 95% of the people facing issues liable for eviction or the need to move out of their housing were able to remain in their residence thanks to the efforts of SET’s social services.
Within the past fiscal year, SET served more than 2,500 residents in 14 public housing high rises through its social services.
