Responsible for the execution of the Parents as Teachers Curriculum to support parents in the role as the first educator of their child, with a focus on the psychosocial needs of families.  The Certified Parent Educator conducts in-person home visits and works intensively with at-risk, pregnant women, new mothers, and infants to improve maternal health, prenatal care, and early childhood health, development, and well-being. This position requires local travel to the home of clients and families.

Reports to:  Associate Director, Home-based Services

The primary responsibility is to take incoming calls of referrals for SET’s programs, ensure that home visiting staff has all of necessary paperwork, and maintaining data in compliance with program expectations.  In addition, there will be administrative duties such as filing, sorting, preparation of reports and memos, and other duties as assigned by the Program Manager.

SET is seeking a full-time Home Visitor case manager to join its Home-based Services team.  The Home Visitor case manager is integral in monitoring and coordinating the health and safety needs of formerly homeless adults and families who have been rehoused in scattered sites, mainly on Milwaukee's near south side.  The right candidate will have a demonstrated knowledge of Social Work theories and practice, with experience in crtitical thinking, conflict resolution and trama-informed care.

SET is seeking a full-time Director of Managed Care to join its Executive team.  The Director of Managed Care is responsible for the direction of all of SET’s managed care programming.  Responsibilities include program monitoring and oversight, long-term planning and managed care business development, marketing and outreach strategies, customer and beneficiary relationship management, health plan contract management, cost, quality and population health performance oversight, compliance, direction of managed care operations, provider relationships and network development, and program budgeting and resource allocation.  The right candidate will possess strong analytical, multi-tasking and prioritization skills; management experience; budgetary and financial skills; excellent verbal and written communication skills.
